今天看到有關中國腸病毒的相關新聞,不禁想到了2003年曾令人聞之色變的SARS。SARS是一種急性呼吸系統感染,是由一種新病原引起的非典型肺炎,發病後病情嚴重,因此取名嚴重急性呼吸道症候群。其特點為發生瀰漫性肺炎及呼吸衰竭,最嚴重會出現瀰漫性肺炎,氧氣交換下降導致肺部缺氧,所以病人會呼吸困難、缺氧,甚至導致死亡。

SARS疫情從中國擴散之後,對周邊國家造成嚴重衝擊,並波及加拿大等較遠的三十餘個國家或地區受害。由於感染病例的時間落差,各國的疫情高峰期先後不一,中國在2002年11月或更早就已出現SARS,但中國官方刻意隱瞞疫情,因為他們擔心會影響到2002年底的中共十六大,以及2003年初的全國人大、政協會議,同時也會降低外商前往投資的意願。然而,紙包不住火,香港爆發特大疫情,死傷十分慘重,其他周邊國家也難逃魔掌。情勢惡化至此,中國才不得不承認疫情的存在,但還是不願把疫情資訊完全透明化。在SARS疫情發展過程,中共隱瞞疫情並阻止世界衛生組織派員調查,其高級官員更公然撒謊,讓世界蒙受威脅,相反的,中國對外的工作重點,反倒是隱瞞疫情讓台灣失去防備,造成台灣民眾的生命、健康遭到空前的威脅,並且全力封殺台灣加入世界衛生組織。

上面這一段敘述,也不過是5年前發生的事,台灣人民是否還記得當時人心惶惶的日子?而現在,相同的情景似乎又要重演,腸病毒71型(EV71)近日在中國安徽、廣東、湖北、浙江等省份相繼爆出手足口病疫情,截至目前為止,中國腸病毒疫情越演越烈,感染人數超過5300人,還有25人死亡。隨著人口流動和氣候條件的配合,疫情有向外擴散的趨勢,包括北京、杭州、陝西、山西等、共八個省市都陸續傳出病例。港澳地區更是緊張,鄰近香港的廣東佛山亦有兩名病例死亡,香港今年也已出現10宗該病感染個案。中國衛生部指出,在阜陽爆發「相當大規模」的疫情後,全國各地的衛生單位必須加強對手足口病的監控。中國衛生部並警告,疫情恐在6、7月達至頂峰。

該型病毒自三月初起就在東部省份安徽阜陽市流傳,但疫情一直到27日才被公開。當中國政府為奧運期間湧入的外籍遊客加緊準備、並開始處理西藏騷亂之際,手足口病的蔓延讓當局感到憂慮。在緊急發布的「手足口病預防控制指南」中,衛生部指出,「為確保北京奧運與殘障奧運順利登場,以及實際維護社會穩定」,有必要防範該疾病的擴散。
中國疾病預防控制中心副主任楊維中,在中央政府網站www.gov.cn線上訪談中表示:「腸病毒71與SARS不能相提並論。SARS是新的傳染疾病,任何人都可能受感染,而且SARS致命率也很高。」地方衛生延遲向大眾公開疫情,在中國媒體間掀起一波激烈討論與批評,認為應該開除地方衛生官員。許多批評人士與中國媒體已要求安徽省相關官員下台負責。

根據美國之音報導,世界衛生組織官員彼得. 柯丁利(PETER CORDINGLEY)認為,中國政府沒有刻意隱瞞安徽省阜陽市爆發的致命手足口病疫情,疫情遲緩公布原因是地方醫療部門診斷緩慢。在此同時,中國的手足口病疫情出現蔓延趨勢,鑑於過去中國隱暪SARS疫情的先例,世衛組織也高度關注。而世衛也研判,目前患者多半是兩歲到五歲的幼童,因此還不至於衝擊到北京奧運賽事。而俄羅斯濱海邊疆區消費者權益保護和公益監督局局長,發函要求中國提供有關黑龍江省是否發現這種傳染病病例的信息。俄羅斯認為疫情訊息流通非常重要,以便俄國知道為防止傳染病傳入俄國境內時需要採取的措施。

5月3日,歐洲台灣醫事聯盟三日致函世界衛生組織秘書長,抗議世衛組織及中國均未及時向台灣通報最近在中國爆發的腸病毒疫情,同時籲請世衛組織及中國尊重全人類共享健康的原則。歐洲台灣醫事聯盟在信函中指出,過去十年來在東南亞及西太平洋地區曾爆發過數次71型腸病毒流行,一九九七年及一九九八年更分別在馬來西亞與台灣造成數以百計的十五歲以下兒童死亡案例,但世衛組織與中國並未正視此一疾病。中國一直到現在才與世衛組織及其他有關國家合作,但進展還是很慢,做法也不適當。

更有甚者,當此一波腸病毒疫情於3月下旬在中國爆發之後,世衛組織及中國都未將此一重要的疾病訊息及時通知台灣。世衛組織則在5月1日發佈的新聞稿中錯誤及不負責任地宣稱,中國將會把此一緊急的健康照護事件通知台灣。歐洲台灣醫事聯盟在信中並對腸病毒可能影響今年8月北京奧運會表示關切。信函中說「我們不希望屆時再有更多的人罹病或死亡。」歐洲台灣醫事聯盟期盼世衛組織與中國政府尊重全人類共享健康的原則,及承認享受健康的權利是基本人權之一,與台灣的衛生主管機關與職業醫護人員合作與互動。

SARS殷鑒不遠,目前中港台接觸已經如此頻繁,再加上隨即而來的三通與開放觀光,今年的腸病毒疫情非常令人憂心。中國預估疫情將在在六、七月達至頂峰,台灣的父母與衛生單位不可不小心以對。目前中國流行的腸病毒是手足口病病毒,會引起發燒、口腔潰瘍、四肢長水泡與紅疹等。傳染途徑是接觸到患者的黏液與糞便,該病毒主要攻擊十歲以下兒童,目前並未有EV-71型腸病毒的疫苗或特效治療,世界衛生組織建議,常洗手、較好的衛生習慣與避免出入公共場所是自保之道。

China on alert over deadly child virus

BEIJING, China (CNN) -- China's Health Ministry strengthened surveillance and dispatched specialists to the eastern Anhui province as the death toll from a virulent virus climbed to 22, the Xinhua News Agency reported.

The number of reported cases of the virus in children rose to 3,736 early Saturday in the hardest-hit city of Fuyang, according to the state-run news agency. There were 3,321 reported cases Friday in Fuyang.

Xinhua reported that 4,529 children have been sickened in 15 cities in Anhui province.

The virus -- called Enterovirus 71, or EV71 -- can cause hand, foot and mouth disease. It's often confused with foot-and-mouth disease in livestock, but the diseases aren't the same, according to the U.S. Centers for Disease Control and Prevention.

An Anhui province official, Wang Yan, told Xinhua that 978 children are hospitalized, including 48 in critical condition.

"Authorities in Anhui ... are doing the utmost to treat the children and strengthen disease control," he said.

The virus is another concern for Chinese officials as the nation prepares to host the Summer Olympics, which open August 8 in Beijing.

Anhui province is south of the Chinese capital.

The Health Ministry issued a pamphlet Saturday urging local health bureaus to step up prevention and control of infectious diseases as the Olympics approach, Xinhua reported.

In milder cases, EV71 can cause coldlike symptoms, diarrhea and sores on the hands, feet and mouth, according to the journal Genetic Vaccines and Therapy.

But more severe cases can cause fluid to accumulate on the brain, resulting in polio-like paralysis and death, according to the journal.

There is no effective antiviral treatment for severe EV71 infections, and no vaccine is available.

Adults' well-developed immune systems usually can fend off the virus, but children are vulnerable to it, according to the CDC.

There was a large outbreak of the virus in Taiwan in 1998 with 78 deaths, and smaller outbreaks recurred in 2000 and 2001, the CDC said.

Wang Jinshan, secretary of the Provincial Committee of the Communist Party of China, visited hospitals in Fuyang on Friday and called for a long-term system to prevent such outbreaks, Xinhua said. In addition, 45 medical specialists have arrived in the city.

Anhui has upgraded its public health emergency status from Class 3 to Class 2, meaning the provincial government can quarantine patients and control movement in to and out of Fuyang.

In a statement Friday, the World Health Organization noted that the number of hospitalized cases has gradually risen since late March, with a sharp increase in the number of cases since April 19.

Public health officials expect the number of cases to peak in June or July, the WHO said.


"Chinese health authorities have put in place targeted prevention and control measures in Fuyang city and Anhui province, including enhanced surveillance, training of health care workers at all levels in treatment, prevention and control, strengthening of environmental health management and the supervision and monitoring of drinking water quality," the WHO statement said.

China tries to control Enterovirus 71 infections

BEIJING - China's Health Ministry ordered heightened efforts to stem the spread of infectious diseases Saturday after an outbreak of a virus that has caused the deaths of 22 children in one city and is spreading.

The outbreak of enterovirus 71 or EV-71 - a type of hand, foot and mouth disease that children are susceptible to - is another headache for the communist government as it prepares for the Beijing Olympics already tarnished by an uprising among Tibetans and an international torch relay disrupted by protests.

Stepped-up vigilance by health bureaus and hospitals to prevent the spread of infectious diseases was necessary "to guarantee the smooth staging of the Beijing Olympics and Paralympics and to ... preserve social stability," according to the order posted on the ministry's Web site.

Prompting the government to act was an unusual jump in cases of EV-71 in Fuyang, a fast-growing city in the rural heartland of central China.

As of early Saturday, 3,736 cases of EV-71 were reported in Fuyang's mainly rural outskirts, a rise of 415 in about 24 hours, health officials said. Besides the 22 deaths, 1,115 people remain hospitalized, 42 of them in serious or critical condition, said officials in Anhui Province, where Fuyang is located.

State-run television footage showed workers spraying disinfectant around houses in rural areas outside Fuyang and medical teams visiting families with small children.

800 cases were reported in other parts of Anhui, the health department said on its Web site. In Guangdong province, 1,000 miles to the south, preliminary tests showed an 18-month-old boy who died Friday was infected with EV-71, and a second suspected death is under investigation, the Xinhua News Agency said.
Outbreaks of hand, foot and mouth infections, not necessarily EV-71, have been reported in at least two other provinces, Xinhua said.

The Health Ministry said it expected infections to climb and peak in June and July. While the order singled out hand, foot and mouth disease for particular concern, it also mentioned hepatitis A, measles and other infectious diseases.

There is no vaccine or specific therapy to treat EV-71. Health experts recommend improved hygiene, with more frequent hand-washing, and disinfecting areas.

The large number of cases spreading across a large area brings up parallels with the communist government's handling of SARS pneumonia in 2003. Government attempts to conceal the emergence of SARS, a new disease at the time, contributed to its spread beyond Guangdong in 2003, ultimately causing 774 deaths worldwide and forcing Beijing to apologize to the world.

A deadly outbreak that raises many unanswered questions hit eastern China in early March, but the government reported its only on 27 April. World Health Organisation is worried by its quick spread. It might be EV 71.

Beijing (AsiaNews/Agencies) – A deadly virus of uncertain origin has killed at least 21 children and infected nearly 3,000 more (one third under the age of eight) in eastern China. First recorded in early March, the outbreak was kept secret by the authorities until last Sunday.

The virus, which might be the enterovirus 71, spreads mostly through contact with infected blisters or faeces and can cause high fever, paralysis and swelling of the brain or its lining. Most of those who died were under the age of two.

A representative of the World Health Organisation said that the situation was worrisome because of the higher than usual mortality rate compared to previous years; it warned parents to keep children away from public places and wash their hands during the outbreak.

The organisation chided China for the delays in informing the public about the outbreak.

But the Health Ministry has come to the defence of local authorities, saying the belated reporting was because medical teams were trying to work out what the illness was.

腸病毒疫情持續蔓延,地方衛生局接獲檢舉,指有小朋友已感染腸病毒,家長仍持續送到學校上課,引發其他家長不滿及恐慌。中央流行疫情指揮中心昨天正式公告強制停課標準,要求今天起各國小和托育機構配合辦理;如果家長明知小孩子已得了腸病毒,仍隱瞞病情送到學校或幼稚園,經勸導不聽,則可能依「傳染病防治法」開罰。

此外,由於已有台中市衛生局等地方衛生單位陸續接到家長檢舉,指有家長隱瞞病情、仍將患了腸病毒的孩子送到學校、安親班、課輔班上課,公衛人員已展開勸說。疾管局強調,「初步仍希望以勸導為主」,但若家長配合度不好,未來也可能依「傳染病防治法」對家長開罰,處三千元到一萬五千元罰鍰。

為了方便家長請假照顧病童,勞委會昨日表示,腸病毒符合請「家庭照顧假」所規定的嚴重疾病,在五人以上公司工作勞工,得請家庭照顧假照顧病童;其請假日數併入事假計算,全年以七日為限。
